Advanced Orthopedics and Sports Medicine is committed to providing patients with the best possible care and experiences while in our facility. Advanced Orthopedics and Sports Medicine believes that part of good health care practice is to establish and communicate a financial policy to our patients. We regard your complete understanding of your financial responsibilities as an essential element of your care and treatment.
Unless other arrangements have been made in advance by either you or your health insurance carrier, full payment is due at the time of service. For your convenience we accept VISA, MasterCard, American Express and Discover.
We are in network with most major insurance plans. For business reasons, we have chosen to be out of network with a few insurance plans. Because many of you have purchased the right to seek care out of your provider’s network, we assure you that we make every effort to honor your choice by keeping our fees competitive. This means that regardless of your insurance plan, or even if you are uninsured, you will receive high quality, professional medical care from one of our highly trained doctors, close to home, at a fair and reasonable cost.
If you have any questions regarding these policies, please discuss them with our office financial counselors. Our counselors are experienced professionals who will review your insurance coverage to show you what costs your plan will cover, and what you will be responsible for.
We have made prior arrangements with many insurers and health plans to accept an Assignment of Benefits. This means that we bill those plans for which we have an agreement and will only require you to pay the authorized copayment or deductible at the time of service. This office’s policy is to collect this copayment or deductible when you arrive for your appointment.
If you have insurance coverage with a plan for which we do not have a prior agreement, we will prepare and send the claim for you. This means that your insurer will send the payment directly to you. Consequently, the charges for your care and treatment are due at the time of service.
In the event that your health plan determines a service to be “not covered,” you will be responsible for the complete charge. Payment is due upon receipt of a statement from our office.
Should you not have insurance you are determined to be a self-pay patient. Payment arrangements will be made with our financial counselor and payment will be made prior to services being rendered.
For all services rendered to minor patients, we will seek payment from the adult accompanying the patient and/or the patient’s parent or custodial guardian.
