If you are a New Patient and have scheduled an appointment, you will receive a request to complete the “New Patient Packet.” Please check your email or mobile phone for this request and complete this virtual packet as soon as possible so we may begin prepping your chart ahead of time to make your visit as smooth as possible. Filling out your paperwork virtually is critical so that we may take proper COVID-19 precautions and provide you a safer, more expedient experience.
