Patient Record Request
Your medical and billing records are maintained by Advanced Orthopaedics & Sports Medicine per Federal and State law. HealthMark Group serves as the Release of Information service provider for Advanced Orthopaedics & Sports Medicine and will fulfill all requests on their behalf. While there are allowable fees for providing copies of your medical records, Advanced Orthopaedics & Sports Medicine and HealthMark Group have elected to waive these fees for all Advanced Orthopaedics & Sports patients.
If you would like to review a copy of your medical records, you may do so via any of the following methods:
• Visit www.HealthMark-Group.com, select “Requestors”, login to the MedRelease tool (note: if it is your first time using this tool, you will need to create an account). Once logged in click “Submit Request” to complete the HIPAA Compliant Electronic Authorization. After the Electronic Authorization Form has been completed click “Authorize Release” at the bottom of the page.
• Complete the Authorization to Disclose, Use or Release Protected Health Information. A copy of this form may be provided by Advanced Orthopaedics & Sports Medicine staff at any location. You may also find this from on the Advanced Orthopaedics & Sports Medicine website at www.advancedosm.com (Select “Patient Resources” – “Patient Forms” – “Medical Record Release”). You may return this form to any Advanced Orthopaedics & Sports Medicine location or submit your request by mail to:

Your records will be released as specified by you in the Authorization form. Within 72 hours, you will receive a notification via email or standard USPS delivery with instructions on how to retrieve your records. To expedite the delivery of your records, please provide your email address on your authorization. If at any time, you would prefer to receive a paper copy of your records you may contact HealthMark Group and ask that a paper copy be mailed to the address on your authorization.
