Request an appointment using our secure online form. Our goal is to contact you by phone by next business day to review your medical and financial information. You can also call our office at (281) 955-2650 during normal business hours, 8 a.m. to 5 p.m., Monday through Friday to schedule.
We are pleased to offer you the convenience of completing and securely submitting your paperwork online. Additionally you could also print forms and eliminate some of the paperwork you would normally fill out in the office before seeing the doctor. If you forget to complete online forms or bring the forms with you, you will have to fill them out in the office before the doctor can see you.

Your medical and billing records are maintained by Advanced Orthopaedics & Sports Medicine per Federal and State law. HealthMark Group serves as the Release of Information service provider for Advanced Orthopaedics & Sports Medicine and will fulfill all requests on their behalf.

I’ve been going to this office to see Dr. Blalock 4 months. He has done 4 different hand surgeries some in February and 1 in April. I probably will need another surgery next year and Dr. Blalock will be my surgeon.
So I was going to get on here and brag about what a great surgeon Dr. Cubbage is. But Cubbage is only 3/4 of the picture. He’ll fix you, but Karen is the one that will care for you and heal you!
Doctor Brooks is the best. His advice to get lubricant injected into my knees has made my life so much better. Kudos!
