Runner’s knee (patellofemoral pain syndrome) is an overuse injury that causes dull pain at the point where the kneecap (patella) attaches to the thigh bone (femur). The pain may be accompanied by swelling and a “popping” or “grinding” sensation during movement. In addition to runners, patellofemoral pain syndrome can affect anyone who participates in activities that involve repetitive knee bending, such as walking, jumping and cycling.

How does a doctor diagnose runner’s knee?

To diagnose runner’s knee, a doctor will typically perform a thorough physical examination and ask questions about where and when the pain and other symptoms first appeared. To confirm the diagnosis, the doctor may order an imaging study, such as an X-ray or MRI.

How is runner’s knee treated?

Mild to moderate knee pain and other symptoms may resolve on their own with time. To accelerate the healing of runner’s knee, a doctor may recommend:

  • Resting the knee
  • Applying an ice pack for 20-30 minutes every 3-4 hours as needed
  • Compressing the knee with an elastic bandage
  • Elevating the knee
  • Taking over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen or naproxen, if needed
  • Performing stretching and strengthening exercises
  • Losing excess body weight (to reduce stress on the knee joints)
  • Wearing supportive shoe inserts (orthotics)

To address very severe runner’s knee symptoms, a doctor may recommend surgery. If necessary, a surgeon can remove damaged cartilage or adjust the position of the kneecap so that the weight load on the knee will be more evenly distributed.
